In the Designer tab:

Click Create. The Create Form Template dialog is displayed.
- Select Agenda Item from the Form Target drop-down list.
- Select a Document Type from the Filter by Document Type Group to narrow the Document Types available for selection.
- Select the Document Type in which you would like to store the forms that originate from this template from the Document Type drop-down list.
- Enter a name for the template in the Template Name field. This field is limited to 60 characters. A value for this field is required.
- Select a color scheme for the form template from the Theme drop-down list.
- If keywords are available for the template, select the Add Keywords check box.

Click OK. The form you created is displayed. The form is empty unless the Add Keywords check box is selected.
