• Medical Record Number (MRN)

• Assigning Authority (if OnBase is configured for overlapping identifiers)

• Patient First Name

• Patient Middle Name

• Patient Last Name

• Patient Date of Birth (DOB)

• Patient Social Security Number (SSN)

• Patient Sex

• Patient Special Instructions

• HL7 Source

Optionally, if the Process Patient Contact Information option is selected the processor can store patient contact information using the following fields:

• Address Type

• Patient Address 1

• Patient City

• Patient State

• Patient Zip Code

• Country Code

• Patient Home Phone

• Patient Mobile Phone

• Patient Email

• Patient Work Phone
