Valid keyword values are required for attachment to patient records. If either the document's MPI or MRN is invalid, the document will not be directly attached to the patient's record, even if the document is attached to a chart.

If OnBase cannot attach a medical record document to a single valid patient, OnBase logs the document as an orphan document. To develop and create reports on orphan documents, use a reporting tool like OnBase Report Services.
