The point at which imported medical record documents are initially attached to patients and charts is configured by the Perform Medical Records Document Attachment drop-down list.

To ensure deficiencies are created during indexing, the On Index option must be enabled. When either On Commit or On Custom Action is enabled, deficiencies are not automatically created, and must be created during the Analysis process.
The Relax Document Attachment Validation (Not Recommended) setting can be used to ignore conflicts between patient and chart records when attaching documents. This setting is not enabled by default, and enabling it is not recommended because it can cause data integrity issues with document attachment.
