If you have sufficient privileges, you can delete a form from the Currently Assigned Forms list. A form can be deleted as long as the associated packet has not yet been completed.
To delete a form:

Click the X displayed next to the form's name. This X is displayed only if you have sufficient privileges to delete the form.
