Depending on your system's configuration, the Integration for Epic MyChart may allow you to view and sign forms in advance of a visit or admission. If this feature is available, the Sign and Review Forms button is displayed on the integration home page.

To sign and review forms:

  1. On the home page, click Sign and Review Forms. The Forms list is displayed.
    Forms may have been assigned in groups called packets. The date displayed after the name of the form is the date the packet was assigned to your record.
  2. Select a form from the list. The following types of forms may be available:

    • Forms to Sign: Forms for you to complete and sign. For information on signing forms, see Filling Out Forms.

    • Forms to Review: Forms for you to review. These read-only forms may need to be signed when you arrive for your visit.

    • Awaiting Approval: Forms that you have signed and saved. These forms are awaiting finalization.

    • Completed Forms: Forms that have been completed and finalized. These forms are read-only.
