While viewing a patient record, you can create new Unity Forms or Image Forms to be associated with a specific patient encounter. To create a new form for a patient encounter, complete the following steps:

Select New Form. A list of patient encounters is displayed:
Encounters are listed in chronological order by Admit Date with the most recent encounters displayed first.

Select the encounter to which the new form should be added and tap Next.
The Form Type list is displayed:
