While viewing a patient record, you can create new Unity Forms or Image Forms to be associated with a specific patient encounter. To create a new form for a patient encounter, complete the following steps:

  1. While viewing a patient record, tap the add (+) icon.
  2. Select New Form. A list of patient encounters is displayed:

    Encounters are listed in chronological order by Admit Date with the most recent encounters displayed first.

  3. Select the encounter to which the new form should be added and tap Next.

    The Form Type list is displayed:

  4. Tap the name of the form you want to create.
  5. Tap OK.
    The form that you selected to create is displayed in a full-screen view.
  6. Fill out the necessary fields on the form. Keyword fields that are part of an AutoFill keyword set can be expanded by entering the keyword that is part of the AutoFill keyword set and then by either tapping outside of the text box or by tabbing to the next field.

  7. When you are finished filling out the form, tap the Submit button to submit the form.
    Note:

    The name of the Submit button may vary based on the configuration of the form.

    CAUTION:

    Depending on the manufacturer or model of your Android device, as well as the camera software being used, photos taken from the device's camera via the Unity Form attachment control may be saved to the device's internal memory or SD card. It is the responsibility of the user or organization to manage or delete these photos from the device after the form is uploaded to OnBase.

    Each form that is submitted to OnBase is indexed with the keyword values related to the patient.

  8. After the form is submitted, you are prompted to create another form.
    • To create another form, select the form to create and tap OK.

    • To return to the patient record, tap No.
