While viewing a patient record, you can create new Unity Forms or Image Forms to be associated with a specific patient encounter. To create a new form for a patient encounter, complete the following steps:

  1. While viewing a patient record, tap the add (+) icon.
  2. Select New Form.

    The Encounters list is displayed:

    Encounters are listed in chronological order by the Admit Date with the most recent encounters displayed first.

  3. Select the encounter to which the form should be added and tap Next.

    The Available Forms list is displayed:

  4. Tap the name of the form to create.
  5. Tap Done.
    The form that you selected is displayed in a full-screen view.
  6. Fill out the necessary fields on the form. Keyword fields that are part of an AutoFill keyword set can be expanded by entering the keyword that is part of the AutoFill keyword set and then by either tapping outside of the text box or by tabbing to the next field.

  7. When you are finished filling out the form, tap the Submit button to submit the form.
    Note:

    The name of the Submit button may vary based on the configuration of the form.

  8. After the form is submitted, you can either create another new form, or you can tap Dismiss to return to the patient record.
    Each form that is submitted to OnBase is indexed with the Keyword Values related to the patient.
