After selecting to sign a form packet, the patient's forms are displayed for completion. The following example shows a Unity Form ready to be completed:

Upon opening the packet, the first form is displayed. A progress indicator is displayed at the bottom of the screen to indicate which of the forms is currently displayed as well as how many forms remain in the packet. In this example, there are three forms in the packet, which are indicated by the three dots at the bottom of the screen. The first form in the packet is displayed, which is indicated by the first solid dot in the row.

  1. Fill out the necessary fields on the form. Keyword fields that are part of an AutoFill keyword set can be expanded by entering the keyword that is part of the AutoFill keyword set and then by either tapping outside of the text box or by tabbing to the next field.

  2. When you are finished filling out the form, tap the Submit button to submit the form.
    Note:

    The name of the Submit button may vary based on the configuration of the form.

  3. After the form is submitted, the next form in the packet is displayed.
  4. Repeat steps 1 and 2 to complete the rest of the forms in the packet.
    After all of the forms have been submitted, the packet is automatically closed. If you set a PIN before signing, you are prompted to enter the PIN.
  5. If prompted, enter the PIN that you set upon entering the packet signing interface and tap Continue.
    The packet is closed. You can review the forms in the packet before finalizing them and uploading them to OnBase. For information, see Reviewing and Finalizing a Form Packet.
