Requester information can be stored in the system and retrieved for later use. It is considered a best practice to do this for any requester not already stored in the system.
To create a new requester:

Do one of the following:

Click the Create Requester button on the request form.

In the Requester Lookup dialog box, click the New button on the Release of Information tab.
The Create Requester dialog box is displayed.

Enter the requester information.
Field
Description
Name
The name of the requester.
Location
The requester's location. This field is required.
Email
The requester's email address.
Relationship
The relationship of the requester to the patient.
Company
The company the requester belongs to.
Phone
The requester's phone number.
Fax
The requester's fax number.
Require Payment
Select to require payment from the requester before the request package is released.
Shipping Address
The address for the destination where the request needs to be shipped.
A shipping address is required.
Billing Address
The address to be used for billing. If the shipping address and billing address are the same, select the Use Shipping Address option. The shipping address information is copied to the billing address fields.
A billing address is required.
- Click Create when you have completed the form. The requester is created.
